An opportunity has arisen for two permanent, full time, Social Worker posts, to work in Community Health and Social Care Direct (CHSCD) for Newcastle City Council.
The posts have become available as 2 staff within our team have been successful in securing promotions within another service area of adult social care, Newcastle.

No two weeks are the same working in CHSCD. Staff members cover a variety of tasks including responding to crisis/emergency situations, completing adult assessments, carers assessments and support planning.
You will provide information, advice and support to prevent the need for long term support and promote independence. The role requires flexibility and team work; Community Health and Social Care Direct Team work in a multi-disciplinary environment with health colleagues and Reablement, as such, the team has an ethos towards collaborative working.
