This post is open to Newcastle City Council employees only.

An opportunity has arisen in the Community Reablement service for 2 Health and Social Care Coordinators (HSCC) on a 37 hour permanent contract, following a 6 week shift pattern inclusive of weekend working.

You will be required to submit an application form and state what skills knowledge and experience you can bring to the role of a HSCC in the Community Reablement service

Relevant recent certified training/qualification specific to the provision of social care (minimum NVQ level 3 or equivalent) or equivalent experience is required.
