This is so that our COVID Control Plan can be correctly implemented and so that we have the latest information about what is happening in the city.
We may need to contact you to confirm further details and we will use the information that you provide to help inform COVID-19 prevention activity and outbreak control.
The details you give will not be published, but may be shared with other organisations, such as Public Health England, to deliver effective COVID-19 controls.
