Community Health & Social Care Direct is a well established team which takes enquiries and referrals for new Adult Social Care cases and known cases in the reviewing system. The work is varied, fast paced and exciting. The team is hugely supportive.
All the work is carried out via the telephone and the role is office based. Office hours are 8am to 5pm and there are shift patterns to ensure all hours are covered.
Communication is a key skill required, both verbal and in writing, as is a knowledge of IT systems. You need to be able to record accurately and concisely and be able to follow procedures and guidance.
The work can be challenging and may involve talking with some very vulnerable people as their first point of contact. The ability to screen calls, make decisions, and ensure satisfactory outcomes is crucial as is the ability to maintain good working relations with colleagues and external stakeholders.
A working knowledge of the Care Act and NMET and its application to referrals is integral to the work we undertake and as such is beneficial.
