We wish to appoint an experienced Social Worker to work within the existing multi-disciplinary team to provide an assessment and care management service within St Oswald’s Hospice. This is a unique opportunity for a highly motivated person to join an innovative team who are committed to the delivery of specialist Palliative care services in hospice and at home. You will have experience of working with adults who have complex needs, and will be required to build upon existing links within the community, monitoring patients, their families and carers post discharge. A recognised counselling qualification would be an advantage, as you will be involved in the provision of emotional and psychological support to patient’s families and carers. Experience in working with people with Palliative care needs would be an advantage. You will have the opportunity for involvement with the bereavement support group and the bereavement support volunteers.
You will be a highly motivated, empathetic and flexible person, with good interpersonal skills, and committed to multi-disciplinary team working. A wide range of training opportunities are available as part of St Oswald’s staff development philosophy. Professional supervision and training are also provided via Adult Services Directorate.
