What is the community rehabilitation service?

After an illness or trauma, it can be difficult for you to get your independence back, especially if you were in hospital for a long time or have tried to cope at home with difficulty.

It might do you good to have a short stay somewhere that is home-from-home, where we can work with you to find out what you need. We would encourage you to practise daily activities, to improve your confidence and ability to manage at home.

These activities may include:

  • Walking and exercise 
  • Personal care
  • Making hot food or drinks
  • Shopping
  • Hobbies or leisure activity

How can you get help from the community rehabilitation service?

You will need to be referred to the Community Rehabilitation Service by a social worker or health professional. If you are already getting help from adult services or health you could ask if they could make a referral for you. You can also ask for an assessment of your needs by the Adult Social Care Direct Team or, if you are in hospital, by the hospital social work service.

Who are we?

We are a team of therapists (occupational and physiotherapists), social workers, residential care staff with district nursing and medical help.

The team has:

A resource manager
The resource manager is responsible for all of your care in the centre. They will oversee your safety and well being while you are in the building as well as that of all other visitors to the centre.

What do senior workers or care officers do?
Our senior worker is known as your “key worker” and is responsible for looking after you. You can choose either male or female trained staff to help you. Your senior worker will work with you and the therapists to write a plan that shows how we can help you get your independence. Your key worker will talk to you often, and to other people who are involved in your care. Our care officers work with senior workers to support you and to make sure we meet all your needs.

What does a social worker do?

Our social worker will look at what you need (we call this an assessment) by looking at what your difficulties are at home and will, if you are ready, arrange support for you to go back home. Our social worker will talk to the person looking after you and take their opinions into consideration. We charge for any service, and look at your service again to check that you are happy with it.

What does a geriatrician do?

The Geriatrician is a hospital consultant who will check your medical needs during your stay. They can make arrangements for further medical assessments or treatment if necessary and will discuss your care with your GP. They will be involved in your rehabilitation to ensure your medical conditions are properly taken into account.

What does a dietician do?

Eating well is important to your health. Your rehabilitation will be helped by you eating a regular and varied diet. You will be asked some questions on admission about your diet, weight and appetite. If you have any concerns regarding your diet the dietician will come and see you.

What does the community pharmacist do?

The pharmacist will carry out a detailed medication review with you during your stay to identify self medicating needs and support. The pharmacist can give you an understanding of the medications you are taking and in some cases can arrange with your GP to have your prescriptions ordered and delivered to your home when you are discharged.

What does a physiotherapist do?

Our physiotherapist will look at your condition and help you to understand what is wrong. They will then work with you to decide what to do. We will take into account your general health and how you live from day to day. You stay in control and the physiotherapist will advise you on how you can help yourself. For example, you they may show you exercises that you can do safely on your own. Where appropriate, physiotherapists advise carers how they can help.

What does our occupational therapist do?

Our occupational therapist will help you to understand your disability or condition and how you can manage everyday tasks. Our occupational therapist will advise you and work with you so that you can lead as full and independent life as possible.

Your family doctor and district nurse

Whilst staying with us, your own doctor and district nurse can still visit you, if the resource centre is within the area covered by them. If this is not possible because of their boundaries, we can arrange for you to register temporarily with a local doctor. We have our own district nurses and they will always visit you if you are staying out of your area.

Visiting

Visitors are welcome at any time, but we would prefer it if your visitors could avoid mealtimes. Please feel free to offer your visitors coffee and tea at any time. You are welcome to invite your visitors for a meal, and we will make a small charge for this.

Charging policy

There is no charge for the Community Rehabilitation Service, however, in line with Adult Services policy should your stay be longer than 8 weeks a financial assessment will be completed and a charge incurred.

Where are we?

The Community Rehabilitation Service is at Connie Lewcock Resource Centre and Chirton Resource Centre. The resource centres are run by Newcastle Adult Services.
